Physiological sigh

Take one breath in through your nose, then a short second sip of air on top, then let a long slow breath out through your mouth. That is one physiological sigh. Two or three of them take about thirty seconds, and it is the fastest pattern on this site.

The pattern, in words

  1. Breathe in through your nose for about three seconds.
  2. Without breathing out, sip a little more air in through your nose — a short second inhale on top of the first.
  3. Let a long, slow breath out through your mouth, roughly twice as long as the first inhale.
  4. Breathe normally for a few seconds, then repeat. Two or three sighs is a typical dose.

What a physiological sigh is

A physiological sigh is a double inhale followed by an extended exhale. You already do it without meaning to — it is the shape of the breath that arrives after crying, and the shuddery one that comes at the end of a long day. Doing it deliberately, a few times in a row, is what researchers call cyclic sighing.

Two things make it useful. It is short enough to do in a queue, in a lift or between two sentences of a meeting, and it is almost impossible to get wrong. There are no counts to track.

How to do it

  1. Breathe in through your nose, unhurried, until your lungs feel comfortably full.
  2. Then sip a little more air in through your nose on top of that breath. It is a short second inhale, not a big one.
  3. Let the breath go slowly out of your mouth — longer than the inhale, until your lungs feel empty but not strained.
  4. Breathe normally for a few seconds.
  5. Repeat two or three times. Stop if you feel light-headed.

The second inhale is the part people skip. It should feel like topping something up rather than starting again.

When to use it

This is the one to reach for when you have very little time or very little capacity: right before you walk into a room, while a phone is ringing, in the first seconds of a wave of anxiety. It is also the gentlest option when a breath hold would feel threatening, which is why it appears on the panic-attack page rather than box breathing.

For a longer, quieter practice — winding down for bed, or a few minutes of deliberate slowing — a counted pattern like 4-7-8 gives you more to hold on to.

What the research says

This is the breathing pattern with the strongest single piece of evidence behind it. In a randomised controlled trial published in Cell Reports Medicine in 2023, Balban and colleagues at Stanford enrolled 108 participants (100 completed) and assigned them five minutes a day for 28 days of either mindfulness meditation or one of three breathwork practices. Cyclic sighing produced a significantly greater increase in positive affect than mindfulness meditation, and the largest reduction in respiratory rate; the benefit grew with adherence over the month[1].

That sits inside a broader literature on slow breathing, which associates breathing under about ten breaths a minute with higher heart-rate variability, more parasympathetic activity and lower reported arousal and anxiety[2]. The NIH’s complementary-health centre still rates much of the relaxation research low or very low quality[3], so the fair summary is: one good trial, a plausible mechanism, and a technique that may help you feel steadier in under a minute. Not a treatment, and not a promise.

Variations

  • Single deliberate sigh — one double-inhale and long exhale, done once. Enough to interrupt a spiral.
  • Five-minute practice — repeated sighs for five minutes, which is what the trial actually tested.
  • Nose-only exhale — quieter in company, though the mouth exhale usually feels more complete.
  • Paired with grounding — one sigh, then 5-4-3-2-1, is a good two-step for a bad moment.

Is this the same as the “Huberman breathing” technique?

It is the same double-inhale, long-exhale pattern. Andrew Huberman is a co-author on the 2023 Stanford trial[1], which is why his name is attached to it online. The technique itself is much older than the study.

How many sighs should I do?

Two or three when you need something quickly. Five minutes if you want the version that was actually tested. There is no benefit to pushing past the point where it feels comfortable.

Why does the second inhale matter?

The double inhale is what distinguishes the pattern from an ordinary deep breath, and it is what the trial tested. If the second sip is uncomfortable, an ordinary slow in-breath with a long out-breath is still a reasonable thing to do.
